Years 4,5&6 – FOOTBALL

Dear Parent / Carer,

Sky Blues will be running an after school Football club from Wednesday 15^th September 2021 for 6 weeks up until October half term.

There are only 16 places available.  You can pay online using SIMS Pay – once the spaces are filled you will no longer be able to purchase a place. Once you have made your payment please complete the form below and post in the box in office area. 

Please make sure your phone numbers are up to date with the school so you can receive all the updates and that we can contact you in an emergency.

This club will take place on Wednesdays and will start on Wednesday 15th September 2021 up to and including 20^th October 2021 at 3.30 pm until 4.30pm. The cost will be £9.00 for the course.

If you give permission for your child to walk home alone after Football club at 4.30pm, year 5 & 6 children only, this must be indicated below; otherwise they will need to be collected promptly. If your child is late being collected or does not attend for two sessions we will remove them from the afterschool club register. 
No refunds can be given.
